The issue looks very uncommon. We are not able to reproduce it as there is no such file in the plugin you provided the link to http://harrytng.ipage.com/wp-admin//austinvietradiowidgets.php
Could you please specify the following so we could understand the possible reason:
1. The operation system you are using;
2. The version of the WordPress;
3. Are you trying to add the Videe.TV widget from Appearance -> Widgets menu?
If no, please specify the steps you are doing this.
4. Are you using a custom theme?

In the meantime I went ahead and started a sample blog for tests with your theme that is running on the same WordPress version 4.4.2. Everything looks fine so far.

As for the issue I’m trying to understand what happens when you click ‘Log In’ depicted on the first image you sent? Do you get ‘Page not found’ error (image #3) after this?
