Jetpack Subscription form not working

Our Jetpack subscription form does not appear to be consistently working. Over the past 5 days we have received 2 comments, 4 emails, and 1 tweet that they have attempted but cannot subscribe. We’ve tested it and have experienced the same problem . . . sometimes. For example, earlier this morning I received an email from someone saying she could not subscribe. I tried to enter her name in the subscription form several times, but had no luck (literally nothing happened). I tried again about an hour later and it worked.
