I put a php form in a WordPress page!

..but I get a Not Found error.

I used the phpexecute plug-in for this. The form has it’s own stylesheet and uses spryvalidations. I can’t figure out what the /runtime.php(42) : eval()'d code is. Please help if you can! I feel I’m really close to making this work.
