Help with HyperLink

Hey guys, Hoping to get some help. At the footer of my site, Prevnt.com, I have 2 hyperlinks, each to a page in my site (a Disclaimer and a Privacy Policy).

The links work fine, MOST of the time. Seemingly arbitrarily, sometimes the links don’t work quite right on some pages and it adds the hyperlink to the existing page address like such:

http://www.prevnt.com/should-we-read-and-listen-less/www.prevnt.com/disclaimer

Any idea why this is happening? I’m no expert, and I’ve had a killer time trying to diagnose this. Any help would be highly appreciated.
