Probably set the page as your home page. Title only displayed on the home page with the default setting (display posts).
I’ve got the exact same problem. No header image from the beginning. I tried creating a new title only page, and setting that as the home page but the header image still doesn’t appear.
Also, nothing shows up in the Bottom1 Home, Bottom2 Home widget spaces, and I’m not getting any Sticky posts or Recent posts showing up.

Hmm, this is very strange. TYPAL Free Theme work properly and tested for WP 3.5+
All I can answer this, repeat these simple steps: activated theme > Settings > Reading Settings > Front page displays: Your latest posts
AND
Appearance > Header
AND
Theme Settings > Heading Box: <h1>Hello, Word!</h1>
Ahhh okay. I had a static page for my home page, so changing over to displaying latest posts made the banner appear.
Yes, thanks dimadinev, I understand the problem now. I don’t want my website to open with my latest posts, so I think I will go on without header. But at least I know what made the header disappear. Thanks.
