GP Not Seeing Page Template

I have the following site:

I have used the GP plugin to add ads to my site. I have added ads to every type of page (Front page, Home, Single Post, Single Page, etc.) using the drop down menu, and of course saved and closed.

I noticed yesterday that new pages (Pages/Add New) do not include any ads.

Here is an example of the “Single Page” option that is working via Publisher (this was the page that showed up in the Manage Ads/Single Page ad placement process):

Here are multiple examples where “Single Pages” are not showing ads:

Here is a test page I just created (Pages/Add New)

None of these pages have the option selected to not show ads.

Couple of additional notes:

1) There are no Adsense ads in the sidebar. Anything you see there are affiliate links.
2) If I move the About Us page to the trash and then “manage ads” with the GP plugin, I no longer have a Single Page choice in the drop down menu. It is as if I have no actual Pages, when clearly I have many Single Page pages.
