    thanks for your question.

    No, something like this is not possible with the plugin. It can not be filled with dynamic data from forms. Sorry.
    You will likely need a custom PHP/mySQL implementation, or maybe try one of the forms plugins that exist already.

    Hi,

    the only forms plugins that I know are Gravity Forms and Contact Forms 7. Unfortunately, I don’t know if these can handle form submissions to a database.
    Also, I’m not available for such professional freelance work, sorry.

    And yes, WP-Table Reloaded can import data from Excel, but only if it is saved as a “CSV” file. Regular XLS files will not work.
