Hello!
Looks like something is messing with the form loading process (more technically speaking it is escaping the string that the form use to load the fields). I don’t think visual composer is doing this but another plugin could be doing this.
To fix this we first would need to find what is messing with the plugin and disable it (or configure it so it doesn’t modify the smart forms string). This could be hard because like i said it i don’t know which plugin is doing this.
I think i could give you a tweak that would bypass this issue (by modifying the smart forms string) but i am not 100% sure, we would need to test it =). If you are interested in this approach please create a support ticket at smartforms.uservoice.com so i can send you the modified file that hopefully would bypass this issue.
