Form is breaking

    Hi, The forms on my client’s website started breaking.
    I can’t seem to resolve it on my own.

    It started happening about a week after some upgrades.
    The forms are in the page I attached and in the contact page as well.
    Would love if you can check it.

    What content do you have in the Form tab panel?

    The problem is not with editing the form.
    The form was created 2.5 years ago and looked fine until yesterday, and now it’s broken.
