    Hi,

    Have you tried changing the “script load method” to direct? (in the troubleshoot area below the list of forms)

    If that doesn’t work then is a conflict with a third party plugin or with the theme.

    A way to identify the origin of the conflict is to deactivate the plugins one by one to check which one is causing the conflict or temporary change the theme if all the plugins are discarded.
