FLV Embed Issues

I recently downloaded the FLV Embed plugin version 1.2.1. I have it installed and when I am using the code instructed [flv:URL Width Height]. On the page where I am using the plugin I am getting the FLV player to show up, and it will say that the flv is loaded 100%, but it just shows the spinning loader for eternity.

I know that the path to the flv is correct, is there something I am missing or are there special FLV publish settings that I need to have?

(Sorry, I cannot link to the project I am working on, I know that makes things more difficult.)
