file upload button missing | WordPress.org

Hi, a have a new wordpress site 3.5.1, I made a form and the file upload button is hidden and there just looks like a text field.

I tried Internet Explorer, Chrome and Safari… none show that button!

the file upload is under PDF word!

Remove the opacity: 0; line.
