feedback page, not posting comments

about a year or so ago I created a page called feedback/comments. The page existed independently from my posts and displayed user feedback/thoughts and whatnot.
However, recently I have not been able to have the comments displayed or have the ability for someone to leave a comment on that page. I’m not sure why the option is no longer appearing or why the comments are no longer appearing.
The page is listed as:
http://www.chazsouthard.org/wordpress/?page_id=133I’m very confused because my dashboard still shows the comments in my database. Does anyone have a suggestion on where to look to fix this problem? a huge thank you in advance to anyone who has suggestions?
