Error creating user

Hello,
When installing the Active directory integration, I encountered a problem. It looks like it works, until creation of the user account in wordpress.
I’m attaching output of test tool on the following link (TEST TOOL SCREEN)
I have all other plugins disabled and twentyfifteen theme activated.
Please Help
