You can use “Custom Fields” addons for this case.

Several questions:
1. Is it any possibility to choose there exactly these additional fields will be shown? I mean it will be better to ask visitor to fill contact form with email, etc. after fill all quizz. It is better for conversion and looks more logical.
2. Other problem with ordering of blocks is that now additional fields shown between test description and main questions… like here: https://studyinfocus.ru/test/nemetskij-yazyk-a1-2/
If these fields used not for special questions but for contact information acquiring it looks really confusing for customers….

But then it will be great to have additional field like label – just to put some text description somewhere before/between or after additional fields to explain reason for these additional fields.
