DB error, 403 error – missing upgrade.php

Hi all,
I’m upgrading from 2.0.4 to 2.1 and have so far followed the instructions to the letter.
Everything that needs installing is installed, and I am getting the basic theme back with a search field in the main body section (no posts, no meta, no blogroll etc)
In the sidebar, this error message appeared:
WordPress database error: [You have an error in your SQL syntax; check the manual that corresponds to your MySQL server version for the right syntax to use near ” at line 1]
And below that:
00 SELECT cat_id
01 cat_name FROMIf I try running the upgrade, I get the following error:
Not Found
The requested URL /wp-admin/upgrade.php was not found on this server.Additionally, a 404 Not Found error was encountered while trying to use an ErrorDocument to handle the request.
Now I went back through and checked the server MySQL and php versions and all are above the required, I have had a good look through the files and as far as I can tell I deleted everything that needed deleting and installed everything that needed installing.
Can someone give me a hand?
