Database Validation & OTP Integration

Hi I am working on e commerce website for medical implants but for that i need to verify every customer is surgeon or not due to medical complaisance. for that we have their Medical council number in the database so at the time of registration i need to verify if their registration number exist on database or not and second thing is i need to verify their mobile number as well i have already bought a SMS package but i dont know how to integrate that with Your registration form. I am new to all this if anyone can guide me a little bit it will be a great help. Thanks in advance.
