Is it possible rolling back to version 3.6.2 allows all your data to appear?
or… did is it possible there is a conflict with your current theme’s CSS? what if you change our theme to the default template, even using your current version of GFD? does that data appear than?

Rolled back to version: 3.6.2 and it seems to show more values, but still a little messed up.

Just found the issue. It is the CSS. The class of “radio” is making them stack instead of inline.
If I replace the CSS class of “radio” with “text” everything lines up perfectly. Thanks for suggestions!
