Customized form results

I need to build a form for a customer that will be a fairly long college football nomination form for the Wuerffel Trophy (similar to the Heisman trophy). They want colleges to fill out the form online and click a submit button and have the results go to 3 different email addresses. I can build the form and setup the results to go to the 3 emails, however, they absolutely require that the form results received in email MUST match the layout/design of their paper copy of the nomination form. We’ve been using formexperts.com for years but found that lately the form software is out of date and they are not going to update it. I have to find a different option. It is more important for the form results to match the application form than the actual online form to match it.
