Customize contact form

Hi!

I can´t make my form that i would like:

Look, my form have the following sections

It should contain 5 textbox:

– Name
– Email
– Phone
– Subjects
– Textbox to the clients type they wants

Also, the form must have a button to upload files and a send button.

I would like customize my contact form like this example:

Name Email

Phone

Subjects

Textbox with the message

Button to upload files

Send button
