create multi-step contact form

I am not sure if I am posting this in the correct place, but here goes:

I am trying to find a plugin, or a script, that will allow me to create a multi-step contact form. I don’t mind if it is a premium item, i just couldn’t locate anything that does what I am looking for for some reason.

To reiterate, I would like to build a contact form for a lead generating website, the form will be devided into 3 phases (or pages), perhaps display alternating animations on each part of the contact form, and then emailing all the information gathered from all 3 forms as a single combined email to a designated address. Any assistance would be appreciated.
