Create an Upload Box

So I have to create something similar to this:

With 8 different images to upload, the placeholder gets replaced when the picture is uploaded and it can be edited (deleted and rotated) once it’s uploaded.

I want to integrate it into a Gravity form I already have done, but doubt it’s possible as there are no addons.

Does anyone know of a plugin that does something like this so I can skip creating this on my own? Even if it’s a paying one. It also doesn’t have to be a wordpress plugin, it can be something else!
