contact form in wordpress

i uploaded the wp-contact-form from
http://ryanduff.net/projects/wp-contactform/
to my test site
http://www.malawigold.com/wordpress/
and all worked fine.

then when i upload the same to my current site
http://www.fulhamslug.com
the whole site is disabled with the error message:
