conditional form display

Thank you for this great form.

I have “songStatus”:radio Button has always default value(IN REVIEW)
I can change it after buying editer plugin (50US$) to accepeted or rejected.

My first question is:
How to disable contact form for logged user since he still has (maximum) 3 rows have (IN REVIEW) values? (he still has 3 songs not changed yet to accepetd or rejected)

second question is: is the editor show me the “songStatus” column with the three values options or only (IN REVIEW) and I have to type the accepted or rejected
