Comments: fields don’t clear after submitting

After a comment is submitted the fields don’t clear for days.
Also after a comment is submitted the message ‘waiting for moderation’ comes up but the comment is published also.

There’s more confusing stuff about the comments but hae to figure out first what exactly is happening…
