Change login form look?

Hello,

I’m using this awesome plugin with myCRED and I noticed on a website that the login form looks like this: http://imgur.com/TTnzzEm.
When I put the login form shortcode on my sidebar it looked like this: http://imgur.com/8T42A41.

So what I need to know is which file do I have edit in order to make my widget look like the first screenshot and also how to add the shortcode to myCRED balance on the form.
