I am using Mailchimp and pasting in the code but it is only showing the email field and not the name field??

Also I can see how to edit the form but how do I change the image?

Hi the lite version only has the email address field, the pro version has first name/last name/ email and phone. To add images you use the wordpress media library there is a tutorial here.
