Can't "insert into post" images

    Hi I have searched this topic and haven’t found a recent answer to this problem.

    This problem started 3 or 4 updates ago, I figured it would be straighten out in a future update but so far it hasn’t.
    I can upload images with no problem they are saved in the media library, the problem starts when I click on “insert into post”.

    Another window opens with this message.

    “This is somewhat embarrassing, isn’t it?
    It seems we can’t find what you’re looking for. Perhaps searching, or one of the links below, can help.”

    In trying to solve this problem I have tried the standard suggestions.
    I have disabled all the plug-ins, deleted unused plugins, tried using a different browser, using twenty eleven theme.

    I had no issues until an WP update about 6 months ago. The only way I can get the image into the post is to write the code in the HTML tab. I would like not to have to do this and just click and insert like before.

    Any more suggestions?
