By post "multi-feedbacks" plugin?

    Hello
    I’m looking for a plugin but don’t really know where to search (feedback, poll, survey). Maybe someone can help.

    I want to write product reviews (one product by post) and want people to easily leave their (multiples) opinions.

    Ex:
    Do you: want this product, have one, had one.
    If you’ve just buy one, quality looks: poor, not bad, very good.
    If you owned this product, are you: deceived, quite satisfied, very satisfied.
    Etc…

    The same form must be displayed on all posts of the same category. The results must be linked to the post.
