Since this is specific to just one site/install, it’s not a WordPress bug. Have you tried:
– deactivating all plugins to see if this resolves the problem. If this works, re-activate the plugins one by one until you find the problematic plugin(s).
– switching to the Twenty Eleven theme to rule out any theme-specific problems.
– resetting the plugins folder by FTP or PhpMyAdmin. Sometimes, an apparently inactive plugin can still cause problems.
– re-uploading the wp-admin and wp-includes folders from a fresh download of WordPress.

Try adding define('CONCATENATE_SCRIPTS', false ); to the bottom of your wp-config.php file (just before the require_once line).
That repaired the problem with the uploaded. Only problem is now, all the wordpress admin is in text format.
Is there a way to rectify this?

Hi! I helped my friend build her wordpress blog some time last year. She’s using a premade themify theme and when she updated her wordpress to 3.3.1, that’s when the problems started. Basically, here are the details:
– Upload/Insert Media button not working at all. When opened in a new tab, the page loads perfectly, even the image file uploads just fine. But after choosing which size the image should be and clicking on ‘insert to post’ button, the page just goes blank.
– She’s already tried deactivating all of her plugins but the problem still persists.
– When she reverted to the twenty ten theme, the Upload/Insert Media button began to work.
– I activated the themify theme again just to see what the problem really was. All of the images uploaded after the update were broken — http://checaparas.com
– When I open any of the broken images’ URL in another tab, this is what shows up: http://checaparas.com/wp-content/themes/bloggie/themify/img.php?src=http://checaparas.com/wp-content/uploads/2011/09/wine1.jpg&w=580&h=210. Apparently, her wordpress was hacked?
– But when I open the original URL of the file (without the height and weight sizes, it shows up just fine — http://checaparas.com/wp-content/uploads/2011/09/wine1.jpg

As you are using a commercial theme, you need to seek support from the theme’s vendors.
