Perhaps the tag names you use on the mail tab are different from the form tab.
Try to change them like this and see if it works.

Thanks very much @orasa, I appreciate your help.
But another form is still giving me issues. The sender’s email address comes with the <[your-email]> at the end.
I would really appreciate your help again please. I took some screen shots so you can see my set up and the error.

Hi @teshuva
You have the same tag name in your form tab for both “your-name” and “your-email”.
Change this one [email* your-name placeholder “Your Email*”]
to this one [email* your-email placeholder “Your Email*”]
What comes after tag type is tag name, and placeholder is a text shown in the textbox as a guideline for the users.

I did not even see that after going over many times. I also discovered the <[email]> that came with the mail was no longer there after I removed if from the mail setting.
