    That’s probably due to a conflict with a third party plugin. Which plugin did you installed before the form started to show in blank?

    To verify that you can deactivate the plugins one by one until the form appears again, this way you will see which one is causing the problem.

    I also recommend you to download the latest version of the plugin since we have made recently some modifications to avoid some of the conflicts with third party plugins. You can download the latest version using your personal download link.

    Excellent, the conflict was with the WordPress Minify’ plugin.
