Admin Panel Errors

I searched on this, but found nothing. For some odd reason my admin panel for wordpress is messed up. On my dashboard my blog itself is showing up in it, at first it appears normal but once it fully loads my blog is there. I also noticed when I try to delete old categories it acts as if it deletes it but when the page refreshes it’s still there.

I deleted and then re-installed the admin panel as well as includes, but still the same thing is happening. Any ideas would be nice.
