Add Theme Shortcode opens empty window

    Hi,
    I’ve just taken over a site which uses OptionTree / Squash version 2.1.4
    When amending pages the ‘Add Theme Shortcode’ button opens a blank window – and does nothing else.
    I’ve tried different versions of PHP and disabled all plugins but still the same.
    Any ideas?
