Add muliple field type

Hi
Using FS Contact Form
Wondering how to create a ‘select” type field
but have different options available within that one set
for example: i would like Customer Contact Preference to be
a) Home Phone ( and a field to enter their number )
b) Cell Phone ( field to enter that number )
c) Text Message (field to enter that number )
OR
d) Email address ( field to enter email address (preferably by adding built in email to verify it )

I want a user to be able to select ONE only and have the appropriate options available upon selection from drop down
