Make sure you are not suffering the dreaded “nested folder” problem. Have a look at this video.

it doesn’t have an index.php or header.php – maybe others

make sure you upload all of the files?
maybe you need to grap a new zip of it.

Where did you get the theme from? Any theme approved by WP has to have both of those files to be workable. Link to theme download?

the zip I just downloaded has all required files.
