Email Notifications full version

I have a client using the full paid version of this plug in, but their support period has expired, so I assume this is the only place I can get any assistance. The issue is – they had a client make an appointment (the default is set to all appointments are PENDING until manually approved), they never received an email that there was a pending appointment (this could be a email fluke with this one, because they’ve been using this software for several months with no issues). However; the problem is, since they did not receive an email, they never confirmed the appointment – BUT, the client got a reminder the evening before the appointment telling them they had an appointment the next day. Is this a bug or is this because the appointment was not manually cancelled – the system just automatically sent out the reminder. It seems it should not do this, since the appointment was never confirmed. Can you help?
