Where’d the State go?

    State/Province is no longer captured in the form. The application form at the link is no longer capturing the applicant’s State/Province. The column is blank in the Submissions table and is missing entirely from the email notification (set to show %all%).

    I’m pretty sure I have made only one change to that field since installing the form. I now have the 2d line address disabled, but that was done weeks ago, and some submissions have come in since then with the State/Province field populated.

    Since most of our applicants are from the USA, we can guess the State based on the City and Zip, but that’s only a workaround.

    Any suggestions?
