Client-side validation not working

I am using WC Fields Factory for a custom form. When one or more fields are left unfilled and the page submitted, I am returned to the incomplete page and given a list of the fields that are not filled out. This all works.

But any fields that I DID fill in are empty. They are not populated with the information I filled out correctly.

In the settings, Client Side Validation is Yes, and On Blur [ + Product Submit ] is also Yes.
