Another blank radio popup

    As of previous posts, the radio button popup is totally blank, I am wondering how this plugin resides in over 100 active installs.

    If you want to keep it, the idea behind it is good, but it needs to be working.

    Just to top-up on this, I do not know why this plugin is there at all. By default the radio button in Contact Form 7 is required, it cannot even be optional, so what is the point out of this plugin?
