Adding extra fields/boxes for comments?

    Using WordPress 2.7 would it be possible to make like three “comment boxes” instead of one, but they would all be one comment of course.

    I’m running a review site and would like to make comments look like a review template.
    Ex
    box 1: The good
    box 2: The bad
    box 3: Overall thoughts
