Is that part of your theme, or a plugin you’re using. If you’re not sure, try deactivating your plugins to explore whether any are responsible.
Just deactivated a captcha plugin and that seemed to fix the captcha problem. Still unsure about the HTML mess

The issue will be easier to debug for someone familiar with the Captcha plugin, so I would recommend posting in that plugin’s support section: https://wordpress.org/support/plugin/captcha#new-post
The person who created the website was using visual composer. The only problem is that he never sent us the licence key and it looks as if he removed visual composer (it isn’t in my plugin list and I am asked for a key). So I will likely have to buy it again. It looks like the captcha has been fixed.
