Setup page not showing

Hi,

I just installed the plugin, it created a few pages, and I was succesfull in changing the css to style the plugin to my need. But I’m missing the page that was supposed to be under:

Setting > Theme My Login

I only see the Theme My Login setup with a sprocket icon in front of it in the backend.
I read that I needed to tick “Show list pages” in the setup page, but since I don’t have that page I can’t make the pages appear in my top menu.

Can somebody point out to me where I can “Show List Pages” for TML of tell me how to set the plugin up correctly?
