Help with comment behaviour

This is a two-part question.
First of all, I have a member who frequently tries to post comments while not signed in and his comments are always held in the moderation queue. The Wordfence setting: “Hold anonymous comments using member emails for moderation” is NOT checked. I (an admin) don’t have this problem if I post while logged out, however this member does share an IP address with another administrator. Could this have something tp do with it? How can I fix this?
Secondly, I have had two reports from visitors unable to post comments in the last few days. One keeps getting a 404 error when submitting their comment and the other keeps getting a message that they must ensure they “have the correct site” before posting. Could these problems be linked to Wordfence? I can find nothing in the site logs that would explain these errors.

It appears right after:
POST /wp-admin/admin-ajax.php
so clearly related to a visitor posting a comment.
