Form Builder with CSV Lookup Capability?

I’m not 100% sure if a form builder is what I need but I’m hoping to get a plugin recommendation here, if one exists.

What I want to be able to do is upload a CSV file with two columns. The first will have a list of names, the second a list of numbers.

I want the names to be able to be added to a dropdown list with the corresponding number from the CSV file added to a separate field along side.

I want to be able to do this more than once. Ie be able to select four different names and have the corresponding four numbers showing along side.

Then I want to be able to calculate the total value of those numbers in another field.

Most form builders I’ve looked at have the calculation functionality but the CSV lookup functionality I’m not yet able to find.
