field data not showing up

All of a sudden emails are showing up with the field names vs the actual data. Any suggestions? I did try creating a new form but that has the same results. There are no errors reported.

So I get the text as it shows in the Mail layout. Like this

From: test <test@test.com>
Subject: [your-subject]

Message Body:
[your-message]
