Contact Form Fields YES vs NO

I have a new website which is not yet live online in development. The contact form 7 will be required to collect profile information in an application. The application will contain fields for street, city, state, and zip of the business owner and as well as the business fields for street, city, state, and zip for the business.

I would like to use a YES vs NO approach, so that if the mailing address information is the same for the business owner and the commercial business, then a check box SAME or a YES vs NO will auto-fill the second address fields so need to fill in the second address if the second address is the same as the Owner. If not, then no change and the address information will be required for both sets of mailing address fields.
