Need help to find the right plugin

Hi

I hope someone can guide me in the righty direction.

I need a plugin where I can set 3 custom search tools to give a specific result.

An example would be to fill in – Name, Age, Date of Birth this would then give the result of Blood type.

I am guessing it would be a database or form of some sort but I can seem to find one that will do the job. The closest I have found are the plugins built to give exam results for students but I need something a bit more configurable. Can someone please give me a little direction, it would be greatly appreciated.
