Custom posts – multi categories

Hi,
Forgive me if it’s a user error but it seems to me that custom posts that are in 2 categories won’t show in both but only one submenu item. Scenario;
Custom post type: Support
Custom post: A new support request
Custom post categories: Cat A, Cat BNow, if I have a top level menu item as ‘Support’ and within submenu items as Cat A, Cat B, the post ‘A new support request’ only shows in Cat A and not in Cat B.
Settings for submenu;
label: …
create submenu containing links: checked
Number of posts: -1
Skip posts: 0
Order by: …
Order: …
Remove original menu item: …
Subtaxonomy posts: Include
Post nav label: %post_titleWould be glad for any help. Capable of development if needed – just point me to the right direction / file.

+ I think I just found out what the issue is…
I just realised there is a ‘Make Primary’ button on the post it self which is a functionality added by Yoast! which you cannot uncheck… See image.

Hi General,
That option is new to me, I haven’t seen it before. I think it would be best to ask Yoast support how to disable that feature (or check if it’s possible to disable it in the Yoast settings).

I have disabled that feature – no difference. Also, should of edited my initial post – Category Posts in Custom Menu plugin is also being used.
I think this has something to do with max_input_vars – haven’t counted the menu items but rough estimate; there are about 100 items. If I remove one item – the other one shows.
I have increased the max_input_vars via htaccess, php.ini from 1k to 10, 20, 50 – still making no difference.
I’ve hit a dead end. Kind of embarrassing too as it is hard to explain to client.
