custom theme-my-login error

Hi
I am trying to add some feilds to the reg form
I added them to register template
nw it says i should add them to theme-my-login-custom.php and add it to my plugins folder. I copied ur file there, every time i rename it and add CUSTOM word, the site stops working with 500 error.

Please advice? The file with ur name is all good. Only after adding custom. And I want to add first name and last name and company name. I found the code for 1st and last name, but not sure how to make the Company name.

If i add these codes to the FUNCTIONS file of my child theme. will that work?
